The Second Hazards Asia Pacific symposium

In 2010 IChemE formed a new partnership with The Chemical Industries Council of Malaysia (CICM) to bring the long running Hazards symposium series to Asia for the first time. As a result, over 250 delegates gathered in Kuala Lumpur, Malaysia last September for the inaugural Hazards Asia Pacific symposium and the meeting was a great success.
The second Hazards Asia Pacific symposium will convene in Kuala Lumpur in April 2013 and both IChemE and CICM are determined to build on the firm foundations that have been laid down. The conference theme will be “Achieving operational excellence in a competitive environment.”

Christina Phang is on the organising committee for this prestigious event and Ian Hamilton will deliver a keynote address entitled Controlling human error for more effective process safety management. Please see the abstract below.
Successful process safety management must begin with a comprehensive assessment of major accident risk. It is widely acknowledged that human error represents the greatest single risk factor, yet the control of the risk posed by human error is often the weakest element of safety management. This paper will describe recent advances in the identification and assessment of human error in safety critical tasks and how this can be applied to improve operational reliability. The paper will call on case study work to illustrate how major accident risk control can be enhanced through an integrated approach to risk management combining technical safety and human factors. It will also show how the analysis can support the establishment of leading indicators of organisational integrity.
